Certain insurance plans require referrals from your primary care physician. Please check with your insurance company to see if a referral is required. The contact number for your insurance company can be found on the back of the card.
It is the patient’s responsibility to check with their insurance company prior to making an appointment to ascertain the procedures and treatments that are covered by the insurance plan. You are responsible for paying the full charged amount for any procedures you agree to, but are rejected by your insurance company once they receive the claim.
As a rule, cosmetic procedures such as laser hair removal, spider vein treatments, and dermal filler and Botox are not covered by your healthcare insurance.
Alpha Dermatology cannot provide any guarantees on whether a specific treatment, even a medically necessary treatment (e.g. removal of a skin cancer) will be covered by your insurance company. Since insurance companies have the ability to change their coverage policies at any time, we have no knowledge if an insurance claim for a specific procedure with a specific diagnosis code will be reimbursed or covered by your insurance carrier, even though that same claim with the came procedure code and same diagnosis code may have been reimbursed in the past.
